Dear Student-Athletes:
It is
time again for all student-athletes to complete the required medical forms
for the upcoming 2008-2009 school year. This letter is to inform all student-athletes
to access the UWF Athletic Training Department's link on the University
of West Florida's Athletic website at www.goargos.com
to print and complete the required medical forms. The following is a checklist
of the required medical forms that must be completed and on file in the
UWF Athletic Training Room before you may participate in intercollegiate
athletic practices or competitions at the University of West Florida:
(Must include parent signature if covered under parents
insurance)
Once again, physicals will be offered on a specified date and time free of charge. Please make it a priority to stop by the athletic training room as soon as you arrive at UWF. All new athletes must receive a physical before athletics participation is permitted. For returning student-athletes, a physical recheck will be performed by the UWF certified athletic trainers and the outcome of that recheck will determine if you need a new physical. Returning student-athletes will also have the opportunity to see a physician if they so wish during physicals. There will also be other medical paperwork that must be completed before you are cleared to participate with your team.
Please make sure all
items are complete and all necessary signatures are provided. Return these
forms to the UWF Athletic Training Department by July 31, 2008.
If you are unable to or do not have the capabilities to print out any
of these forms please contact the athletic training room at (850) 474-2142
and we will gladly mail you hard copies of the forms for you to complete.
Your cooperation in
returning the required paperwork is greatly appreciated. Please contact
us with any questions or concerns.
Also enclosed is a list of NCAA banned substances that the NCAA and the
UWF Athletic Department will be testing for during your tenure at UWF.
The UWF Athletics Drug Testing Policy will be reviewed with you at a later
date on campus.
